Applying to Five Branches (Bridge)

Once you have made the decision to enroll in the DAcHM/DAc Bridge Program you are ready to begin the application process. To be considered for admission to Five Branches University, the following items are required:

Use your full legal name on all admissions documents.

Digital photograph, passport style and size

A non-refundable $100 application fee payable by check or money order to Five Branches University. Please indicate which program you are applying to.

An up-to-date resume listing all previous education, job experience, accomplishments and volunteer work

From all colleges and/or universities attended, including Master’s and graduate level coursework.  Academic transcripts must be emailed or mailed directly from the academic institution to the attention of the Santa Cruz admissions office.

Transcripts from prior undergraduate or TCM education completed in another country must be reviewed by a credential evaluation agency that is a member of the National Association of Credential Evaluation Services. Transcripts need to be evaluated on a course-by-course basis and include the total number of earned credits and cumulative GPA. The evaluation should be sent directly to Five Branches University by the evaluating agency. To locate an appropriate agency, please visit www.NACES.org. Applicants who have completed TCM courses overseas are advised to utilize the services of the Educational Records Evaluation Service, Inc.

Please convey to  the Application Committee what inspired you to study Chinese Medicine, what have you been doing with your practice as a Licensed Practitioner and why you feel the Five Branches University Bridge Program is a good fit for you to move forward.
