While enrolled, students may request unofficial copies of transcripts for personal review. Students may request only one transcript per semester. There is no charge for this service. For the form click here.
Students and graduates may, at any time, request official copies of transcripts.
Official transcripts can be ordered at any time by submitting a signed Transcript Request Form (Click Here) to the Registrar’s Office via mail, fax, or email scan.
The request must contain the following information: student name (current and at time of attendance), contact information (address, e-mail, etc.), last date of attendance, recipient information, and student signature.
Transcripts will not be released if a student has any outstanding financial obligations to the University (including fines accrued from overdue or missing Library material, parking violations and clinic fees).
The Family Educational Rights and Privacy Act (FERPA) of 1974 protects the privacy of a student’s education records by placing limits on who may have access to the records, what information may be shared or disclosed, and how that information may be used.
Five Branches University complies with FERPA and has strict policies and procedures in place governing student records. In general, the University does not access a student’s records or disclose information to anyone (including students’ family members) without the student’s written consent. The University maintains all records required by the California Reform Act at its primary administrative location for a minimum period of five years.
